5 common mistakes with Name Change Certificate (Chor 3 / Chor 5) — and how to avoid them
These are common problems with Name Change Certificate (Chor 3 / Chor 5) submissions. Checking them before lodgement reduces the risk of a request for further information.
1. Submitting expired evidence (police clearance and medical certificates expire after 12 months)
2. Using a self-translation or non-NAATI translator — DHA and AHPRA reject these immediately and request a re-submission
3. Incomplete scans of the source document (missing back pages, stamps, or signatures), forcing a redo of the work
4. Failing to translate stamps and signatures — DHA returns the file as 'incomplete translation'
5. Inconsistent name spellings across documents (passport vs. birth certificate) — usually requires an additional Statutory Declaration
Acceptance is decided by the receiving authority; a correct translation does not guarantee an outcome.
Documents typically translated together (story bundle)
